Severe Mental Illness (SMI) Health Checks

We need to ensure that the physical health of our population, particularly those with severe mental illness, is protected. This includes ensuring they receive physical health checks and can access their free flu and covid vaccinations if eligible.

This service offers our SMI population additional access to a range of healthcare professionals including pharmacists and nurses providing bespoke care and planning based on the needs of the patient. The health check includes:

  • Basic obs including height/weight/pulse/BP
  • Pathology including HBA1c, and lipid profile and prolactin where appropriate
  • Review of alcohol/smoking status and brief lifestyle advice
  • Review if diet and lifestyle measures including illicit drug use and exercise and brief lifestyle advice
  • Ensure compliance with national screening programmes
  • Review of medications

 

Diabetes Clinical Support and Intervention

A clinical review is undertaken by a clinical pharmacist for all patients following the HCA 8 care process check.

The clinical review allows discussion and appropriate action to be taken based on the findings of the initial face-to-face ‘information gathering’ check. This is specifically for people with type 2 diabetes and is intended to take pressure off GP practices while providing additional choice and flexibility to patients.

The review includes:

  • discussing results, implications and trends since the patient was last reviewed
  • specific enquiry regarding medication adherence, tolerability and side effects
  • starting/stopping/adjusting medication for managing glycaemia, blood pressure and lipids in line with applicable NICE guidance, including setting individualised targets as appropriate
  • appropriate evidence-based management of identified complications of diabetes 
  • arrangements for onward referral of appropriate urgency
  • signposting to other services

Diabetes 8 Care Process Checks

The Evexia team are working collaboratively with Luton GP practices and their Primary Care Networks to identify patients with diabetes that require a Health Care Assisstant (HCA) check to review the following parameters:

  1. HbA1c measurement (blood test)
  2. Cholesterol measurement (blood test)
  3. Creatinine measurement (blood test)
  4. Urinary albumin testing (urine test)
  5. Blood pressure measurement
  6. Weight measurement and BMI
  7. Smoking status recording
  8. Foot examination

 All 8 care processes are conducted by a suitably qualified and competent HCA.

 

Community Lipid Clinic Service

The Community Lipid Clinic accepts referrals of people meeting the relevant clinical criteria who are registered with a GP practice in Bedfordshire, Luton and Milton Keynes ICS

The primary aim of the service is to provide an equitable, effective and efficient care pathway, to support identification of Familial Hypercholesterolemia (FH) and optimisation of lipid lowering therapy in people at very high risk of CVD.

The Community Lipid Clinic provides:

  • a standardised, evidence-based treatment pathway (including monitoring of effectiveness and tolerability) for patients 
  • person-centred, high quality care
  • care efficiently, with clinical contacts designed for maximal clinical impact
  • onward referral of people with uncontrolled hypercholesterolaemia despite progression through the treatment pathway to the specialist lipid services
